BMJ Case Rep Published online: [please include Day Month Year] doi:10.1136/ bcr-2014-206553 DESCRIPTION We report the case of a male newborn, the second triplet of non-consanguineous healthy parents, born from a dichorionic triamniotic pregnancy with adequate prenatal care and normal ultrasounds. Spontaneous labour occurred at 32 weeks of gestation and a caesarean section was performed. The Apgar score was 5/6/8, birth weight was 1395 g and no malformations were noted. After sectioning the umbilical cord for venous catheter placement, an abnormal structure with a lumen discharging watery liquid was noted (figure 1). The abdominal ultrasound was normal, so a catheter was passed through the lumen and intestinal content was aspirated. Surgical correction, with resection of the duct, took place on the fifth day of life (figure 2). Histology revealed proximal intestinal mucosa consistent with patent omphalomesenteric duct (OMD). The newborn regained normal bowel function and resumed breastfeeding after 5 days. The OMD is an embryological structure that connects the midgut to the yolk sac and normally regresses between the fifth and ninth weeks of fetal development. Remnants of the OMD may persist in approximately 2% of the population, the most common presentation being Meckel’s diverticulum. Symptoms occur most frequently during childhood years and vary greatly, as patients may be asymptomatic, have abdominal pain, painless rectal bleeding, or intussusception/prolapse of ileum at the umbilicus, among others. An accurate diagnosis and management of this condition, with a highly unusual presentation, is important as it may lead to invagination of the small bowel and intestinal obstruction, with a significant increase in mortality.

Coexistence of multiple omphalomesenteric duct anomalies.
The omphalomesenteric duct is an embryonic structure which connects the yolk sac to the midgut. The omphalomesenteric duct attenuates between the 5th and 9th week of gestation. Failure of the omphalomesenteric duct involution, either partial or complete, results in various omphalomesenteric duct remnants including Meckel's diverticulum, patent vitelline duct, fibrous band, sinus tract, umbilica...
متن کاملPatent Omphalomesenteric Duct with Protruding Bowels through a Ruptured Omphalocele
Persistent omphalomesenteric duct is a rare congenital anomaly and protrusion of the bowel through the duct is the only neonatal emergent clinical entity associated with this condition. The pathogenesis is the lack of intestinal involution by the ninth gestational week. There are several associated anomalies like intestinal malrotation, oesophageal atresia and omphalocele, which are all extreme...
متن کاملA Case Report of a Patent Omphalomesenteric Duct Presenting with Meconium Discharge from the Umbilicus
This report describes the case of a newborn presenting with a patent omphalomesenteric duct remnant fistula identified at birth with meconuim in the umbilical cord. The infant had no antenatal care and presented to the NICU in mild respiratory distress and meconium within the umbilical cord. We instituted a brief work up and proceeded to surgery where the remenant was resected. The patient did ...
متن کاملUltrasonographic diagnosis and minimally invasive treatment of a patent urachus associated with a patent omphalomesenteric duct in a newborn
RATIONAL Patent urachus (PU) is due to an incomplete obliteration of the urachus, whereas patent omphalomesenteric duct (POMD) is due to an incomplete obliteration of the vitelline duct. These anomalies are very rarely associated with one another. We describe a case of a newborn with a PU associated with a POMD, who was diagnosed by an abdominal ultrasound (US) and laparoscopy, and managed with...
متن کاملIntestinal prolapse through a persistent omphalomesenteric duct causing small-bowel obstruction.
Persistent omphalomesenteric duct as a cause of small-bowel obstruction is an exceptional finding. A neonate presented with occlusion due to intestinal prolapse through a persistent omphalomesenteric duct. Remnants of the duct were successfully resected, and the postoperative course was uneventful. We discuss the presentation of omphalomesenteric duct and its management.
